We suggest that you first print this web page, in order to have available the notes to help you fill in Form 288a.

Click on the link below to view Form 288a.  Then print out the form by clicking on the printer symbol near the form's top left corner.  [DO NOT try to print the form by using your browser's print facility (for example with Internet Explorer, do not click on File and on Print)].  Complete the form manually, before posting it back to us.  

You will need a copy of Form 288a for each Director of the company, and also one for the Company Secretary.  If the Secretary is also a Director, only one form is required for this person, and in such a case, there must be at least one other Director appointed, for whom a separate Form 288a will be required.

NOTES ON COMPLETING FORM 288a

Complete the following items for each Director and Secretary:-            

  • The "Appointment as" box(es)                              
  • Full name and address details   
  • Consent signature box, indicating the capacity in which you are signing

    • For each Director, also complete the following items:-

  • Business occupation                                             
  • Date of birth                                                          
  • Nationality                                                            
  • All other directorships held in the past five years  

PLEASE DO NOT DATE THE FORM,
(apart from date of birth for Directors), 
AND DO NOT SIGN IN THE "Signed" BOX. 

LEAVE ALL OTHER BOXES FOR FORMITRITE TO COMPLETE

Return the form to Formitrite, NOT to Companies House
